Aspergilloma due to Previous Pulmonary TB
This is a truly amazing case.
The patient was previously treated for a multidrug resistant TB. As a complication of his TB the patient developed a large right upper lobe cavity (upper image above).
The patient remained asymptomatic for several years but then developed fever, cough and hemoptysis.
A repeat CT chest (bottom image) showed the presence of debris in the previous TB cavity. His bronchoscopy specimens were positive for Aspergillus species making it a likely diagnosis of aspergillosis.
Subsequently, the patient required lobectomy.
