The elderly patient presented with diffuse abdominal pain and hematemesis. CT abdomen and pelvis showed evidence of diffuse intestinal ischemia with air present in the intestinal wall. What remarkable about this case is the amount of air the patient had in the biliary system – pneumobilia (circled on the image above). The air could also be seen in the portal system.
Not surprisingly, the patient did not survive hospitalization.
